6.5.2 ISSUE AND RECEIPT {Page 2}

• The Acceptor of the Safety Document shall sign the Acceptor part of the Safety Document and take it into safe custody together with the appropriate items issued and listed on the Safety Document.
• A Safety Document shall be personally retained when it is:-
• a Permit (ACCESS),
• a Permit (ROSH),
• a Permit (TEST),
• a Permit (LIVE),
• on HV Apparatus,
• when identified in local management procedures or
• when the Safety Controller preparing the Safety Document decides it is necessary.
