• Must ensure the Acceptor understands:
• The limits of work
• The further precautions identified by the Safety Controller
• The items issued with the document.
• Must ensure that all items listed are available to be issued (usually only applies to personally held ‘single’ working party documents).
• Some sites allow both Initial {Front of safety document} and Reissue {Rear – Transfer Record (or additional sheets thereof) or via Record of Further Acceptors (ROFA) form(s)} - some sites limit ‘non’ Safety Controller Issue to Reissue only – check you local arrangements.
• Likewise at some locations ‘non’ Safety Controller Issuers cannot issue/reissue any Permit ROSH, Permit TEST or Permit LIVE – check you local arrangements..
• If in any doubt STOP and refer to the duty Safety Controller.
